Anyone wishing to become a United States Citizen must have an immigration physical performed by a civil surgeon. Dr Suparna Chhibber MD, is designated a civil surgeon by the Bureau of Citizenship and Immigration Services. She is licensed to conduct Immigration Exams and Physicals for the Department of Homeland Security.
Please note that Immigration Physical Exam is not covered by your insurance.
Included in the immigration physicals:
- Basic Physical Examination
- Blood work required by USCIS including Tuberculosis (TB)
- Vaccination status – verification of required immunizations
- Administration of appropriate and needed vaccinations (including; Tetanus, Measles Mumps & Rubella, Varicella, Hepatitis B, and Influenza)
- Copy of laboratory results
- Certified and sealed I-693 form
What we need from you;
- Filled out patient information sheet (at office)
- Government issued PHOTO identification card (drivers license or passport)
- Immigration Physical form, I-693 – only first two (2) pages completed with your information
- Proof of Vaccinations
